Minnesota State High School League Eligibility Brochure 
Holy Trinity JH/JV Volleyball players, please fill-out and return pages 10 & 11 to HT's athletic director. HLWW athletes, please fill-out this from on HLWW. This form needs to be completed once a year.

2024-2025 Sports Physical Form
You must have a sports physical on file before you can participate in any sport. A new physical will be needed every 3 years and before participating in your first practice. Please turn in sports physicals for all sports to HT's athletic director. She will share with HLWW. 

Holy Trinity School Athletics Consent and Liability Waiver
For Holy Trinity JH/JV Volleyball athletes only. Must be turned in prior to participating in the first day of practice or first tournament.

For Holy Trinity JH/JV Volleyball athletes only. Please turn in on the first day of practice. 

Transportation Authorization and Waiver
During the school year, a shuttle is provided to HLWW for students to take to practice. Students are highly encouraged to use this shuttle. If students opt to drive themselves or other HT students to practice, each of the occupants in the vehicle must fill out and submit this form to HT's athletic director prior to the start of every seasonal sport. This is a seasonal form, so if a student fills out this form they may not use the shuttle for transportation to practice for the duration of the season. For any questions, please contact the athletic director. 

Travel Release Form
All athletes are required to ride the team bus to and from all athletic events. This travel release form is for unique circumstances only. Please note that HLWW has their own travel release form. HLWW's form does not protect Holy Trinity School. Holy Trinity families must fill out this form every time a parent/guardian is taking a student-athlete to or from any athletic event. Failure to turn-in a travel release form to HT's athletic director prior to an athletic event will result in the student-athlete having to take the team bus home. Travel release forms must be turned in no later than 9 AM the day of the athletic event to allow communications between HLWW coaches and Holy Trinity.
